If you are a patient and have not registered for our patient portal, contact us at (631) 941-2000, opt 5.  If you are a patient and have already used our previous portal, please log in with your old user name and password, you will be prompted to select new challenge questions.  If you cannot remember your old password, please contact our office at (631) 941-2000, opt 5, and we will need to re-establish you with our new portal.
